GTCYS Symphony Tour: Consent, Authorization and Release

THIS CONSENT, AUTHORIZATION, AND RELEASE (the “Consent”) is provided to the Greater Twin Cities Youth Symphonies (“GTCYS”) in connection with its 2024 Symphony Tour of France and Spain from June 15 – 25, 2024 (the “Tour”).
-The term “Student” refers to the above-named student.
-If Student is at least 18 years old on June 15, 2024, the term “Consenting Parties” refers to Student and their parent(s) and/or legal guardian(s).
-If Student is less than 18 years old, the term “Consenting Parties” refers to Student’s parent(s) and/or legal guardians.

The Consenting Parties acknowledge that all pertinent information concerning the Tour including individual itineraries and transportation arrangements and such other matters as might affect their decision with respect to Student’s participation in the Tour have been adequately disclosed to the Consenting Parties and that they consent to Student’s participation in the Tour.

Student is voluntarily participating in the Tour. The Consenting Parties understand that there are risks associated with participation in this Tour including, but not limited to, physical and/or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability, death or economic loss. Such injuries or outcomes may arise from GTCYS or others' actions, inactions, or negligence, or the condition of the Tour location(s) or facility(ies). The Consenting Parties assume all risks of Student’s participation in this Tour, whether known or unknown to us, including travel to and from the Tour venues (including air travel) or any events incidental to this Tour. The Consenting Parties agree that they have been given sufficient opportunity to ask questions and obtain information about the programs and measures being taken to promote health and safety. INDEMNIFICATION AGREEMENT
The Consenting Parties release GTCYS of all claims arising from Student’s participation in the Tour and agree to indemnify, protect and save GTCYS harmless against and from any and all damages, losses, liabilities, obligations, claims, costs and expenses (including, without limitation, attorney fees and court costs) of any kind or any nature whatsoever which may at any time be imposed upon, incurred by or asserted or awarded against the GTCYS arising in whole or in part out of Student’s participation in the Tour. This release is without reservation of any kind except liabilities arising out of any wrongful acts on the part of GTCYS, its directors and employees.

This Consent shall be binding upon Student and their parent(s), or guardian(s) whose signature(s) appear below and their respective assigns, and inures to the benefit of GTCYS, and its successors and assigns.

This Consent shall in all respects be governed by, and construed and enforced in accordance with, the laws of the State of Minnesota.
